A Clinical Study of Chronic Otitis Media with Cholesteatoma




Abstract
Cholesteatoma is a dangerous disease because it causes destruction of surrounding bones including ossicles and induce hazardous otologic and intracranial complications.
Authers performed the clinical study in 104/463 cases of chronic otits media with cholesteatoma operated from January 1991 to December 1994 at department of otolaryngology of Paik hospital, Inje University.
@ES The results were as follows:
@EN 1) The total cases of operation was 463 of which 104 cases (21.9%) were cholesteatoma .
2) Sex ratio was 1.2 : 1, mal predominance, and age incidence was the highest in third decade as 34.6%.
3) The method of operation were as follows: open cavity mastoidectomy with tympanoplasty in 58 cases (55.8%), intact canal wall mastoidectomy with tympanoplasty in 30 cases(28.8%). Besides intact bridge mastoidectomy and atticotomy were done.
4) Postoperative follow up audiogram showed that loss of hearing (10-30 dB) in 51 cases (49.0%), and gain of hearing (10-20dB) in 40 cases(38.5%).
5) The most common site of facial nerve dehiscence was the tympanic portion in 35 cases (87.5%).
6) The site of labyrinthine fistula was mainly lateral semicircular canal(SCC) alone(24 cases, 88.9%), 1 cases of poterior SCC(3.7%) fistula and 1 cases (3.7%) of coexistence of lateral and posterior SCC dehiscence was noted.
